Dr. Jonathan Mawere is the Administrator and Chief Operating Officer of the Queens Boulevard Extended Care Facility. He brings to the C-suite an extensive educational background, including a master of healthcare leadership degree from Brown University where in 2017, he was awarded an achievement of distinction in his capstone research work on electronic health records in post-acute care. Dr Mawere earned doctorates in both physical therapy and medicine at the MGH Institute of Health Professions and Xavier University School of Medicine respectively which combined with his license in administration equips him well to serve a complex patient population. The American College of Health Care Administrators (ACHCA) awarded Dr Mawere the prestigious Eli Pick Facility Leadership Award in 2016 in recognition of his team’s achievement of dimensions of organizational quality that few others have been able to reach. Dr. Mawere was honored by the Brooklyn United Methodist Church Home with the 2015 Community Service Award for his outstanding leadership and dedication to the Healthcare industry and community at large. Under Dr Mawere’s leadership, Queens Boulevard Extended Care Facility became a high performing organization with accomplishments including its CMS 5-star rating for 9 consecutive years, consistent US News and World Report ‘s honor roll listing of Americas best Skilled Nursing Facilities, CareScout facility rating of #1 in New York State for several years and best (AAA) in Queens County as well as the Joint Commission Accreditation. Queens Boulevard has earned preferred provider status and affiliation agreements with leading health Systems in New York, Health plans and other partners due to its superior patient outcomes.
This 280-bed skilled nursing facility treats a diversity of patients and has been successful in reducing re-hospitalization rates for both subacute and long-term residents though evidence-based practice. Queens Boulevard has upgraded its services and adopted best practices improving its ability to treat advanced illnesses at the facility. Queens Boulevard has optimized care access and quality through state-of-the-art onsite radiology, laboratory and specialty services.
Recently, Queens Boulevard completed the development of an inpatient and outpatient state-of-the-art dialysis unit that will help to improve care capacity within the community for patients who need renal services.
